RN Field Case Manager to assist with safe and early RTW by attending appointments, communicating results and findings and coordination of care as approved. Providing appropriate information for education of treatments and documentation of compliance and outcomes to determine progress.TWO openings
1-North of Chicago, IL Spanish strongly preferred
1-Near border of Wisconsin & Very North of Chicago, IL Spanish strongly preferred
Fundamental components but not limited to the following:
60-75% Travel to MD appointments or therapy appointment to determine treatment progress and RTW plan.
10-15% report creation and emails (clerical) updating accounts of CL responses and MD treatment and RTW plan
10 % communication with CL, ER, therapist for compliance and progress
2 years RN clinical practice experience
Ability to travel within a designated geographic area for in-person case management activities
Bilingual Spanish preferred
Excellent analytical and problem solving skills
Effective communications, organizational, and interpersonal skills.
Ability to work independently (may require working from home).
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
Effective computer skills including navigating multiple systems and keyboarding
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.
